Jaczko and other NRC staffers incorrectly believed that there was no water left in the No. 4 reactor, leading to the call for a 50-mile zone. "If this happened in the U.S., we would go out to 50 miles," Bill Borchardt, the agency's executive director for operations, said in discussions with Jaczko on March 16.
